In 2015, 4,043 eligible and critical access hospitals reported to the Medicare EHR Incentive Program, according to ONC data.

Here are the top health measures electronically reported to a public health agency.

1. Immunization registry: 93 percent

2. Reportable laboratory results: 88 percent

3. Syndromic surveillance: 76 percent

4. Specialized registry: 4 percent
