San Francisco-based Dignity Health plans to implement athenahealth's population health service platform athenaCommunicator Enterprise.
The population health platform uses cloud-based services,software and analytics to gain insight into populations and help coordinate care, control utilization and improve outcomes.

"These goals require the innovative application of technology and services to make large-scale population health management a reality."
