Through a new partnership with the Society of Hospital Medicine, Cerner is integrating a new readmission prevention technology into its EHR.
Cerner clients will be able to access the Society of Hospital Medicine's Project BOOST toolkit from their Cerner EHRs. The toolkit will be incorporated into Cerner's Readmission Prevention Solution.
Project BOOST is a national initiative from the SHM that provides expert mentoring and peer support to help hospitals improve care for patients as they transition from the hospital to their homes.
Project BOOST provides automated early identification of specific readmission risk factors and corresponding interventions, preparedness checklists to help patients identify concerns about leaving the hospital, patient-centered discharge instructions and a "teach back" education process to help patients understand information related to their hospitalization and discharge.
"The enhanced readmission solution provides our clients with access to nationally recognized content and an industry-leading predictive model to help identify and manage patients that are at risk for readmissions," said Bharat Sutariya, MD, vice president and CMO of population health at Cerner.
