The following data breaches occurred over the past eight weeks and were reported by Becker's Hospital Review, starting with the most recent.
1. The personal information of approximately 3,300 patients may have been compromised after a group of employees at Boston-based Partners HealthCare engaged with phishing emails.
2. Los Angeles County-USC Medical Center notified approximately 900 patients treated at the medical center's Augustus F. Hawkins Mental Health Center that their medical records were removed from the hospital without authorization by an employee.
3. Records of patients of Veterans Affairs Long Beach (Calif.) Hospital containing personal health information were found improperly disposed in a dumpster by the hospital.
4. Austin, Texas-based Seton Family of Hospitals, a division of Seton Healthcare Family, notified approximately 39,000 patients that their protected health information may have been accessed by hackers through a phishing scheme.
5. A physician practice affiliated with Bangor-based Eastern Maine Healthcare Systems notified 1,200 patients that their email addresses were accidentally exposed.
6. Advantage Dental, a dental group based with clinics across western Oregon, notified 151,626 patients that their data may have been compromised in a recent data breach.
7. Premera Blue Cross, a health plan based in Mountlake Terrace, Wash., reported a cyberattack on its IT systems that has compromised the data of 11 million customers, employees and business affiliates.
8. Mosaic Medical, a nonprofit community health system with locations throughout central Oregon, reported a break-in incident at a health IT department in a temporary administrative office in Bend.
9. Sacred Heart Health System in northwestern Florida reported a security breach due to a phishing attack on a third-party vendor that possibly exposed the information of 14,000 patients.
