Four physician practices and organizations have selected Chicago-based health IT vendor Allscripts to provide its chronic condition management platform and its value-based care team to help organizations apply for status as an accountable care organization.
Heart and Vascular Center of Lake County in Gurnee, Ill., selected Allscripts' Chronic Care Management Program to help boost long-term patient engagement and overall health improvements for patients with chronic conditions.
Santa Ynez Valley Medical Associates in Solvang, Calif., and Frischer Medical Group in Downey, Calif., selected the CCM Program, too.
Additionally, the Londonberry, N.H.-based Southern New Hampshire Primary Care IPA chose Allscripts' Value-based Care team to help them apply to become an ACO.
