Healthcare technology platform PointClickCare Technologies is partnering with referral management platform Findhelp to integrate social determinants of health information into a platform available for clinicians and social workers.
The partnership aims to help clinicians ensure that a patient's foundational needs such as food, clean water and housing are met, according to an Oct. 13 news release from PointClickCare. Research from the National Academy of Medicine shows that social determinants of health make up more than 80 percent of a patient's health outcomes.
"Patients receive immense benefits from their clinicians having access to this socially relevant information in real time," Findhelp founder and CEO Erine Gray said in the release. "Our partnership with PointClickCare will ensure that care teams have the most accurate view in their patient assessment workflow to better direct patient discharge planning, support the patient journey through access to social services, and reduce readmissions."
