Mid-year review: 56 ACOs formed so far

Below is a list of 56 accountable care organizations and collaborative care agreements formed since January 1, beginning with the most recent.

1. Methodist Health System ACO joins Catalyst Health Network
Plano, Texas-based Catalyst Health Network and Dallas-based Methodist Patient-Centered Accountable Care Organization announced MPCACO will join Catalyst's affiliated network.

2. The Memorial Hospital to launch ACO with Western Healthcare Alliance
The board of The Memorial Hospital, based in Craig, Colo., unanimously approved a recommendation from CEO John Rossfeld to become an ACO through Western Healthcare Alliance, a Grand Junction, Colo.-based alliance of rural hospitals.

3. UnitedHealthcare, Advocate expand coordinated care initiative
UnitedHealthcare and Downers Grove, Ill.-based Advocate Health Care plan to expand their ACO beginning July 1 from 5,500 Medicare Advantage beneficiaries to more than 80,000 UnitedHealthcare members with employer-sponsored plans. 

4. Catalyst Health Network adds Aetna
Catalyst Health Network, a clinically integrated network of primary care physicians based in Plano, Texas, will become the first physician network in the area to establish value-based agreements with the top four payers.

5. Jefferson Health renews accountable care partnership with Independence Blue Cross
Philadelphia-based Thomas Jefferson Health System and Independence Blue Cross reached a new multi-year agreement effective July 1 to offer Independence beneficiaries access to covered services at several Jefferson Health facilities.

6. Humana, Rockford Health partner for accountable care in Illinois
Humana and Rockford (Ill.) Health reached an accountable care agreement that went into effect May 1.

7. Humana, Lancaster General launch ACO
Lancaster (Pa.) General Health and Humana reached an accountable care agreement effective April 1 to improve the healthcare experience for more than 3,000 Medicare Advantage beneficiaries.

8. BCBS launches 4 more intensive medical homes in Illinois
Blue Cross and Blue Shield of Illinois launched four intensive medical homes April 1, bringing the statewide total to 14. The IMHs were launched with Illinois Critical Access Hospital Network in Princeton, Lurie Children's Health Partners in Chicago, Rush Health in Chicago and Unified Physicians Network in Skokie.

9. UnitedHealthcare, WakeMed Key Community Care launch accountable care partnership
Raleigh, N.C.-based ACO WakeMed Key Community Care and UnitedHealthcare reached a partnership to improve care coordination and health services for the more than 175,000 beneficiaries on UnitedHealthcare's employer-sponsored plans who are patients of a WKCC physician.

10. 5 Dallas health systems combine ACO efforts
Five healthcare systems based in the Dallas-Fort Worth area announced the launch of a joint ACO, comprised of 42 hospitals, 72 outpatient facilities, more than 1,300 primary care physicians and more than 4,100 specialists.

11. Providence Health & Services Western Montana and Sisters of Charity of Leavenworth announce joint venture: 3 things to know
Providence Health & Services Western Montana and Denver-based Sisters of Charity of Leavenworth Health System formed a new joint venture to create a coordinated care network in Montana, called the Northern Rockies Healthcare Alliance.

12. HCA North Texas joins ACO
Catalyst Health Network, a Plano, Texas-based ACO, added HCA North Texas, the 15-hospital Irving-based branch of HCA, to its affiliate network.

13. Florida Blue, Tampa Bay Integrated Healthcare Network form ACO
Florida Blue and TampaBay (Fla.) Integrated Health Network formed a physician-centric accountable care program in the TampaBay area.

14. Cigna, Regional Cancer Care Associates partner for value-based care
Cigna and Hackensack, N.J.-based Regional Cancer Care Associates are launching a value-based initiative to improve care for patients receiving chemotherapy.

15. UnitedHealthcare, Palo Alto Medical Foundation to launch ACO
UnitedHealthcare and Mountain View, Calif.-based Palo Alto Medical Foundation inked a deal to launch an ACO for more than 63,000 beneficiaries enrolled in employer-sponsored UnitedHealthcare plans.

16. Cigna, Northwestern Medicine ink collaborative care deal
Cigna and Chicago-based Northwestern Medicine Physician Partners reached an agreement for a collaborative care initiative similar to an ACO.

17. Aetna, CHI Health form ACO in Nebraska
Aetna and Omaha, Neb.-based CHI Health announced the launch of an ACO for employers, the state's first commercial, product-based ACO.

18. Palm Vascular Centers to join ACO
Miami Beach, Fla.-based Palm Vascular Centers will join the Fort Lauderdale, Fla.-based PremierMD ACO, according to the medical center's CEO Eric Rogers.

19. Aetna, Value Care Alliance to launch new accountable care agreement in Connecticut
Aetna and the Value Care Alliance launched an ACO aimed at improving the coordination and delivery of care for more than 40,000 Aetna commercial health plan members in Connecticut.

20. Cigna launches 3 collaborative care agreements in Texas
Cigna finalized three collaborative care initiatives effective April 1 with the following three medical groups: Plano, Texas-based Catalyst Health Network, Dallas Methodist Physicians Network and Dallas-based Genesis Accountable Physician Network.

21. Aetna, Emory Healthcare to partner on accountable care
Atlanta-based Emory Healthcare and Aetna announced plans to form a new ACO to improve care coordination and delivery.

22. Highmark, Lancaster General Health to launch accountable care arrangement
Highmark Blue Shield and Lancaster (Pa.) General Health formed an accountable care arrangement to improve care coordination and health outcomes for 27,000 Highmark commercial and Medicare Advantage members.

23. Kettering Health Network, BCBS partner on patient-centered primary care program
Anthem Blue Cross and Blue Shield in Ohio, and Dayton, Ohio-based Kettering Health Network reached a new value-based partnership to launch BCBS' Enhanced Personal Health Care program at Kettering-owned practices.

24. UnitedHealthcare, Baylor Scott & White Quality Alliance to collaborate
UnitedHealthcare plans to give 52,000 UnitedHealthcare members with employer-sponsored plans access to the Dallas-based ACO Baylor Scott & White Quality Alliance.

25. HackensackAlliance expands ACO to 10k Aetna members
Aetna and HackensackAlliance ACO in Hackensack, N.J., announced the launch of a new accountable care agreement to expand coordinated care to more than 10,000 Aetna members, effective April 1.

26. BCBS of Illinois launches 4 new ACOs, 4 more to come
Blue Cross and Blue Shield of Illinois formed four new ACOs in the Chicago area with Chicago-based Presence Health, Arlington Heights, Ill.-based Alexian Brothers Health System, Evanston, Ill.-based North Shore University Health System and Kane County IPA in Tinley Park.

27. Wellmark BCBS launches 4 new ACOs
Wellmark Blue Cross and Blue Shield launched four new ACOs in and near Iowa. The new ACO agreements were formed with West Burlington, Iowa-based Great River Health Systems, Mercy Iowa City (Iowa), Omaha, Neb.-based Methodist Jennie Edmundson Hospital and Pella (Iowa) Regional Health Center.

28. Truckee Tahoe Medical Group joins rural ACO group
Truckee Tahoe Medical Group joined the National Rural Accountable Care Consortium, a program designed to help rural providers participate in the Medicare Shared Savings Program.

29. UnitedHealthcare, Baylor Scott & White Quality Alliance to collaborate
UnitedHealthcare shared plans to give 52,000 UnitedHealthcare members with employer-sponsored plans access to the Dallas-based ACO Baylor Scott & White Quality Alliance.

30. Heritage Provider Network, Fresenius Medical Care to build coordinated care networks
Marina Del Rey, Calif.-based Heritage Provider Network and Waltham, Mass.-based Fresenius Medical Care, a vertically integrated renal company, announced an agreement to collaborate and develop coordinated care networks and population health management services throughout the country.

31. Integrated Health Network adds SSM Health, grows to 5,700 providers
Integrated Health Network of Wisconsin, based in Brookfield, is welcoming St. Louis-based Catholic nonprofit SSM Health to its ACO to strengthen coverage in the south central region of Wisconsin.

32. Health Choice Preferred ACO, Regence BCBS of Utah to collaborate on value-based care
Health Choice Preferred, a subsidiary of Franklin, Tenn.-based IASIS Healthcare, agreed to collaborate with Regence Blue Cross and Blue Shield of Utah on a value-based care initiative that will serve approximately 10,000 beneficiaries enrolled in Regence's PPO plans.

33. 700 physicians to form ACO in Portland
The Portland (Ore.) InterHospital Physicians Association announced plans to launch a new ACO called Care Connect Northwest. Twelve physician organizations, with approximately 700 primary care physicians and specialists, plan to participate in the new ACO.

34. Humana, Integrated Health Network to launch accountable care agreement
Brookfield-based Integrated Health Network of Wisconsin agreed to partner with Louisville, Ky.-based payer Humana on a three-year accountable care agreement for Humana's Medicare Advantage beneficiaries.

35. Aetna, Virtua Medical Group to launch ACO
Aetna and Marlton, N.J.-based Virtua Medical Group announced the launch of a new ACO that will expand on an existing Medicare program.

36. Catalyst Health Network ACO to add 20 outpatient clinics
Catalyst Health Network, a Plano, Texas-based ACO, announced a new partnership with Dallas-based Questcare Medical Clinics and its Dallas-based parent company, Questcare Medical Services.

37. Humana, HealthSpan Physicians announce accountable care agreement
Humana announced Tuesday a new multi-year accountable care agreement with HealthSpan Physicians, a network of 200 primary care physicians based in Cincinnati.

38. Cleveland Clinic launches Medicare Shared Savings ACO
Cleveland Clinic announced it will launch a Medicare Shared Savings Program ACO to augment its current value-based care initiatives, which include care paths and a patient-centered medical home.

39. Summa Health, Humana sign accountable care agreement
Humana and Akron, Ohio-based Summa Health System's existing accountable care organization announced an agreement to provide coordinated, cost-effective care for Humana Medicare Advantage beneficiaries.

40. Largest payer, provider in Kansas partner for new ACO
BlueCross BlueShield of Kansas and the Via Christi Healthier You Alliance in Accountable Care have signed a multi-year, value-based agreement, effective Jan. 1.

41. BCBS of Texas forms first ACO-like organization with state medical association
BlueCross BlueShield of Texas and the Texas Medical Association are partnering to launch a first-of-its-kind initiative: an ACO-like organization with a medical association, rather than a hospital or health system.

42. Cigna-HealthSpring expands value-based agreement with Texas ACO
Cigna-HealthSpring expanded an agreement with Dallas-based Genesis Accountable Physician Network, the ACO of Genesis Physicians Group.

43. Cigna, Austin Regional Clinic partner for collaborative care
Cigna and Austin (Texas) Regional Clinic launched a collaborative care agreement, similar to an ACO.

44. Iowa Specialty Hospital joins rural ACO group
Iowa Specialty Hospitals & Clinics joined the National Rural Accountable Care Consortium, an organization that makes it possible for rural and critical access hospitals to participate in the accountable care delivery model.

45. BCBS extends MissionPoint partnership 2 years
BlueCross Blue Shield of Tennessee and Nashville, Tenn.-based MissionPoint Health Partners agreed to extend their accountable care network agreement two years and expand its offerings.

46. Cigna launches 3 more collaborative care agreements
Cigna formed three more collaborative care agreements with Durham, N.C.-based Duke Connected Care, Dallas-based Texas Clinically Integrated Network and Portland, Ore.-based Providence Medical Group.

47. 3 medical groups join Texas ACO
Amarillo (Texas) Legacy Medical Accountable Care Organization expanded its network with 82 providers from BSA Health System's three physician groups located in Amarillo, Texas.

48. UnitedHealthcare, Catalyst Health Network form ACO
UnitedHealthcare and Plano, Texas-based Catalyst Health Network plan to create an ACO for UnitedHealthcare employer-sponsored health plan beneficiaries in the Dallas-Fort Worth area.

49. Cigna forms 5 new collaborative care agreements
Cigna announced the launch of five new collaborative care agreements in Florida and California, which began Jan. 1 and emphasize preventive health through better patient access and more coordinated care.

50. BCBS of Texas launches 3 new ACOs
Blue Cross Blue Shield of Texas launched three new physician-led ACOs on Jan. 1, in partnership with Austin (Texas) Regional Clinic, Plano, Texas-based Catalyst Health Network and Austin, Texas-based Southwest Provider Accountable Care.

51. Humana, Boulder Community Health to partner for accountable care and population health
Boulder (Colo.) Community Health formed an accountable care agreement with Humana to provide more coordinated care for Boulder County residents with Connect for Health Colorado plans and Humana HMO plans.

52. Empire BCBS, Mount Sinai launch new ACO
Empire BlueCross BlueShield and New York City-based Mount Sinai Health System started an accountable care agreement, effective Jan. 1.

53. Virginia Gay Hospital to join National Rural Accountable Care Consortium
Vinton, Iowa-based Virginia Gay Hospital joined the National Rural Accountable Care Consortium, an organization that helps rural hospitals participate in the accountable care model.

54. Humana, Aurora Health enter accountable care agreement
Humana and Milwaukee-based Aurora Health Care formed an accountable care agreement for Humana Medicare Advantage patients, effective Jan. 1.

55. Health Net, John Muir Health to form ACO
Health Net of California, a division of Woodland Hills, Calif.-based payer Health Net, announced the launch of an ACO with Walnut Creek, Calif.-based John Muir Health.

56. Humana, United Hospital System enter accountable care partnership
Humana and Kenosha, Wis.-based United Hospital System signed a three-year accountable care agreement starting Jan. 1.
