Cornerstone Health Care, a Highpoint, N.C.-based physician group with more than 365 physicians, has partnered with Blue Cross and Blue Shield of North Carolina to form an accountable care organization.
As part of the agreement, Cornerstone will work with BCBSNC customers to coordinate and manage their care on an ongoing basis with health navigators and patient care advocates, according to the news release. BCBSNC will share data with Cornerstone, and the two organizations will work together to reduce unnecessary hospital admissions and emergency room visits.
With this agreement, Cornerstone Health Care now has value-based contracts with all of its payors. Details of this contract's value-based metrics and reimbursement plan were not released.
This is BCBSNC's fourth ACO agreement. It is effective May 1.
