Five Pioneer accountable care organizations in Massachusetts are working together to improve post-acute care quality and reduce spending, according to U.S. News & World Report.
The five Pioneer ACOs are Steward Promise in Boston, Partners HealthCare in Boston, Newton.-based Atrius Health, Mount Auburn Cambridge Independent Practice Association in Brighton and Westwood-based Beth Israel Deaconess Care Organization. The ACOs, along with post-acute care provider representatives, are creating standardized guidelines on how patients discharged from a hospital should be treated when they need some level of post-acute care, according to the report.
Reducing variation in post-acute care can save Medicare and the ACOs money as well as improve quality of care and patient experience, according to the report.
