Buffalo, N.Y.-based Dent Neurologic Institute announced that an administrative clerk accidentally emailed the personal information of 10,000 patients to approximately 200 patients.
The information did not contain any medical diagnoses, Social Security numbers or financial information but did contain patient names, addresses, email addresses, scheduling codes and names of primary physicians.
The institute has self-reported the event to HHS and will send a letter notifying those affected.
