Humana will launch two programs to better coordinate kidney disease management for patients in four states, the insurer said Dec. 11.
Through new agreements with Nashville, Tenn.-based Monogram Health and Somatus in Vienna, Va., Humana Medicare Advantage members in Louisiana, Mississippi, Georgia and Virginia can access coordinated kidney disease management, in some cases in their own home.
Under the agreement, nephrologists, nurses, dietitians and social workers with Monogram and Somatus will coordinate members' care with their primary care physicians and other providers to improve their quality of life.
The agreement with Monogram has already started, and the one with Somatus begins Jan. 1.
