Albany, N.Y.-based health insurer Capital District Physicians' Health Plan is investing more than $1.4 million to bring 70 more physician practices into its enhanced primary care model, the insurer's version of patient-centered medical homes, according to a Business Review report.
CDPHP's medical home program now encompasses 146 physician practices and about 170,000 members covered by the insurer, according to the report.
Most of the practices are around Albany and surrounding communities.
